Job Summary:
We are seeking a dedicated and compassionate Support Worker to join our team at our residential service in Barton-upon-Humber. As a key member of our team, you will be responsible for delivering person-centred care, promoting independence and confidence in daily living skills, social interactions, and community engagement. You will work closely with our multi-disciplinary team to develop and implement Positive Behaviour Support plans, ensuring the individual's safety and well-being at all times.

Responsibilities:
* Provide person-centred care, promoting independence and confidence in daily living skills, social interactions, and community engagement
* Work closely with our multi-disciplinary team to develop and implement Positive Behaviour Support plans
* Assist individuals with autism, acquired brain injury, and complex mental health needs with daily living tasks, social interactions, and community engagement
* Build strong relationships with individuals, families, and colleagues, ensuring effective communication and collaboration
* Participate in regular team meetings and contribute to the development of individualised care plans
