Care Support Coordinator

About the Role
We are seeking a Care Support Coordinator to join our Magic Moments Team. As a key member of the team, you will play a vital role in supporting residents to lead fulfilling lives.
The successful candidate will have a genuine interest in promoting independence and lifestyle opportunities for the elderly. You will be responsible for assisting in the assessment of the needs of the residents, providing stimulation and exercise through a wide and varied activities programme, and building relationships with residents and families to gather life story information.
Key Responsibilities
- Assist in the assessment of the needs of the residents, through meeting with them, team members and family members
- Support providing stimulation and exercise to residents that meet their individual needs through a music, exercise and arts & crafts programme
- Build relationships with residents and families to gather life story information and ensure care plans and activities are developed and updated accordingly
Requirements
- Confident to deliver group and 1 to 1 activities
- A team player who engages well with others
- A strong communicator
- Energetic and hard-working
- Knowledgeable of basic IT Skills
