Key Responsibilities- Support Occupational Therapists and Physiotherapists in the delivery of rehabilitation services
- Conduct non-complex assessments and develop treatment plans under the direction of therapists
- Work autonomously and as part of a team to achieve patient-centered outcomes
- Communicate effectively with patients, families, and healthcare professionals
- Assistant Practitioner qualification or equivalent
- NVQ level 3 or equivalent
- Evidence of continued professional development
- Knowledge of long-term conditions, mental illness, and disability
- Experience of rehabilitation in the community
- Ability to work independently and as part of a team
