About the Role: We are seeking an experienced Team Leader to join our Home Care Assessment Reablement team (HART) in a challenging and rewarding role that involves working with Occupational Therapists and Physiotherapists to deliver reablement plans to help service users regain their independence.

Key Duties:

  • Undertake first visits, including risk assessments, and provide day-to-day line management support to Reablement Support Workers.
  • Conduct Care Act Assessments where an ongoing eligible need has been identified.
  • Work collaboratively with health colleagues in hospital settings and the community.

Requirements:

  • Relevant experience working in adult social care settings and good understanding of the Council's Adult Social Care strategies.
  • NVQ 3 or equivalent and relevant experience working with adults in a social care setting is essential.
  • Able to communicate clearly and accurately with a wide range of people, including representatives from other agencies.
