Social Care Coordinator - Disability Services Job Description

We are looking for a skilled and experienced Social Care Coordinator to join our team in Bury. This is a full-time position that offers a competitive salary of £40 per hour.

As a Social Care Coordinator, you will be responsible for working with vulnerable children and their families. You will develop and monitor outcome-focused plans to improve the lives of these children.

The ideal candidate will have knowledge and/or experience in safeguarding and working with looked-after children. You will be passionate about outcomes for children with disabilities and committed to building relationships with children and families to effect positive change over the long term.

Key Responsibilities:

  1. To work with team members to develop innovative solutions for complex care plans and assessments for families.
  2. To participate in duty management to ensure there is appropriate cover at all times.
  3. To chair CIN meetings, planning meetings, Core Groups, and PLO meetings as required.
  4. To accept responsibility for learning by attending training courses and taking advantage of staff development opportunities.
  5. To contribute to progress in establishing and embedding child-focused ways of working with children, young people, and families throughout our work.
