We are seeking a qualified Social Worker to join our team at Monmouthshire County Council for an initial 3-month period. This part-time position is based in Chepstow Hospital and involves working 2.5 days a week on care planning, assessments, and reviews.
Key Responsibilities- Manage a complex caseload of clients with secondary mental health needs
- Conduct capacity act assessments
- Participate in best interest meetings

The team provides support to individuals over 65 who come under secondary mental health services under the Mental Health (Wales) Measure 2010, as well as individuals under 65 diagnosed with dementia.
Requirements- Experience working with older people with mental health needs (or generic adults experience)
- Relevant Social Work Qualification
- Registration with Social Care Wales
- Enhanced DBS registered on the update service or be happy to apply for a DBS
- 3 Years of referencing
- Driving license with access to a car
- 20 years' experience of the Social Work market
- Local agency based in Cardiff
- Welsh-speaking consultant available
