Job Overview:

We are seeking an Experienced Adult Social Worker to join our team in Kingston upon Hull, providing comprehensive social work assessments and care plans for adults with complex needs.

About the Role:

  • This is an exciting opportunity to work within a dynamic and supportive team, providing high-quality social work services to vulnerable adults.
  • You will be responsible for assessing and developing care plans, as well as offering advice, advocacy, and support to service users and their families.
  • Excellent communication and interpersonal skills are essential for this role, as you will be liaising with external agencies and stakeholders on a regular basis.

Key Responsibilities:

  • Provide social work assessments and develop care plans in collaboration with multidisciplinary teams and stakeholders.
  • Offer advice, advocacy, and support to service users and their families, empowering them to make informed decisions about their care and wellbeing.
  • Liaise with external agencies to coordinate holistic care packages and ensure continuity of support.
  • Maintain accurate and up-to-date records, ensuring confidentiality and data protection standards are upheld.

Requirements:

  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working within adult social care settings, ideally within a multi-disciplinary team environment.
  • Sound knowledge of relevant legislation, policies, and procedures relating to adult safeguarding, mental capacity, and care management.
  • Commitment to continuous professional development and willingness to engage in ongoing training and supervision.

Estimated Salary: £35,000 per annum
