Key Responsibilities
  • Provide high-quality care and support to patients in their own homes, promoting independence and well-being.
  • Work collaboratively with our multidisciplinary team, including physiotherapists, occupational therapists, and nurses, to deliver comprehensive care.
  • Develop and maintain effective relationships with patients, families, and carers, ensuring their needs are met and their voices are heard.
  • Contribute to the development and implementation of individualized care plans, ensuring patient-centered care and outcomes.
Requirements
  • 12 months' experience of working in a care setting, with a strong understanding of patient-centered care.
  • Experience of working under distance supervision, with a focus on autonomy and decision-making.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Good general education, with a willingness to undertake further training and development.
