Ambulatory Clinical Pharmacist

The Ambulatory Clinical Pharmacist plays a vital role in providing high-quality, patient-centered care within the Care New England Health System. As a key member of the interdisciplinary team, this pharmacist is responsible for ensuring the safe and effective use of medications, promoting evidence-based practice, and optimizing medication regimens to achieve desired outcomes.
Key Responsibilities- Functions as the medication expert on the care team, providing evidence-based medication recommendations and ensuring adherence to optimal therapy.
- Reconciles and assesses medication regimens for adherence and optimal therapy, working collaboratively with the care team to achieve desired outcomes.
- Provides medication and disease state management education to patients, empowering them to take an active role in their care.
- Practices within established collaborative practice agreements, consistent with services delineated within the agreement.
- Participates in quality improvement initiatives, contributing to the development and implementation of best practices.
- Collaborates with providers and patients to ensure high-quality, cost-effective care, promoting a culture of excellence within the organization.
- Documents all encounters within the electronic health record, communicating effectively with the care team to ensure seamless care coordination.
- Contributes to the achievement of contractual obligations, including comprehensive medication reviews, adherence outreaches, and targeted medication conversion initiatives.
- Actively participates in the Pharmacy Committee, driving innovation and excellence in pharmacy practice.
- Develops presentations on quality metric data and pertinent clinical topics, sharing knowledge and expertise with the care team.
- Participates in the development of new programs, services, and best practices for pharmacy and medical teams, fostering a culture of collaboration and excellence.
- Precepts and contributes to the development of pharmacy students, mentoring the next generation of healthcare professionals.
- Assists in the development and maintenance of the PGY1 and PGY2 residency program, supporting the growth and development of pharmacy residents.
- Actively participates in care retention initiatives, working collaboratively with the care team to ensure high-quality, patient-centered care.
