About the Role

We are seeking a highly motivated and enthusiastic Community Mental Health Nurse to join our team at Aneurin Bevan University Health Board. As a key member of our Community Mental Health Team, you will be responsible for providing high-quality, user-centred care to individuals with complex, severe, and enduring mental health problems in a community setting.

Key Responsibilities
  • Work as part of the Multi-Disciplinary Team to assess, plan, and implement care for patients with mental health needs.
  • Undertake core CMHT duties such as routine and urgent assessment clinics, assessing, planning, and implementation of care.
  • Develop and maintain effective relationships with patients, families, and carers to ensure the delivery of high-quality care.
  • Contribute to the development of adult mental health services in Torfaen and participate in quality improvement initiatives.
  • Take on the role of Care Coordinator for a number of patients and work collaboratively with other healthcare professionals to ensure seamless care.
