We are seeking a highly motivated and enthusiastic Community Mental Health Nurse to join our team at Aneurin Bevan University Health Board. As a key member of our Community Mental Health Team, you will be responsible for providing high-quality, user-centred care to individuals with complex, severe, and enduring mental health problems in a community setting.
Key Responsibilities- Work as part of the Multi-Disciplinary Team to assess, plan, and implement care for patients with mental health needs.
- Develop and maintain effective relationships with patients, families, and carers to ensure seamless care coordination.
- Contribute to the development of adult mental health services in Torfaen, ensuring the delivery of high-quality, evidence-based care.
- Undertake core CMHT duties, including routine and urgent assessment clinics, and participate in the implementation of care plans.
To be successful in this role, you will need to possess:
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Knowledge and experience of community mental health care, with a focus on delivering high-quality, user-centred care.
- Ability to work autonomously and as part of a team, with a strong commitment to delivering excellent patient outcomes.
- Desirable: ability to speak Welsh, with a willingness to learn and develop language skills.
