Health and Justice Navigator

We are seeking a skilled and compassionate Health and Justice Navigator to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our team, you will be responsible for identifying health and social care needs of individuals in custody and providing support to navigate these services.
Key Responsibilities- Screen individuals for health and social care needs using relevant tools and databases.
- Assist individuals in accessing health and social care services, including referrals to mainstream services and other interventions.
- Complete Court Reports outlining individuals' needs and provide information to key decision-makers in youth and criminal justice agencies.
- Maintain effective communication with individuals, relatives, carers, and professionals to ensure seamless service delivery.
- Actively seek support and supervision to maintain safe practice and facilitate robust decision-making.
- Professional recognized qualification such as RMN, RGN, Learning Disability Nurse, Social Worker, Occupational Therapist, AHP.
- Knowledge of mental health issues and/or learning disability.
- Knowledge of use and interpretation of health screening tools.
- Knowledge of assessing and mitigating risk, including risk of harm to self and others.
- Knowledge of a wide range of vulnerabilities, including the needs of children and young people.
- Knowledge of appropriate health and social care interventions.
- Knowledge of relevant law, including Mental Health Act, Mental Capacity Act, Children Act, Social Care Act, and PACE code C.
- Knowledge of Safeguarding Adults and Children.
- Knowledge of Information Governance, Data Protection, and GDPR.
