Beauchief Medical Practice is seeking a part-time Mental Health Practitioner to join our multidisciplinary team. The successful candidate will work one session a week, between 4 and 6 hours, engaging with both health and community working.

Main Duties of the Job

Key Responsibilities:

  • Manage patient groups in line with General Practice procedures and risk management protocols.

Professional:

  • Assess needs and identify problems relevant to the care of people aged 18 and above.
  • Develop care plans in partnership with individuals and carers.
  • Implement care programs to ensure high standards are achieved.
  • Review care effectiveness and initiate actions where necessary.
  • Advise and support individuals and carers to promote health and well-being, and prevent illness.
  • Recognize situations detrimental to health and well-being, and initiate required actions.
  • Maintain effective communication with agencies involved in care, both in hospital and community settings.
  • Ensure professional registration and adherence to statutory codes of practice and continued professional development requirements.

Administrative:

  • Maintain accurate patient care records in accordance with professional codes of conduct and Practice procedures, ensuring records are available when required by relevant agencies.

  • Current NMC Registration
Experience
  • Experience working in a mental health setting.
  • Demonstrated skills in assessing and managing individuals with mental health problems.
  • Experience working in a primary care setting.
