Key Responsibilities- Deliver personalized care to patients in their own homes, including personal care and support to help them stay at home.
- Work as part of a team to provide care to patients at the end of life, supporting their families and loved ones.
- Take the lead role on shift, ensuring the dignity, comfort, and care of patients and families.
- Provide leadership to Community Support Workers on shift, ensuring effective handover and continuity of care.
- Build rapport quickly with patients and families, gaining confidence and trust to deliver care in a patient's home.
- Assure continuity of care through effective handover, ensuring that you have picked up the necessary information to deliver care in a patient's home.
- Observe and report changes in symptoms, condition, and circumstances, entering this information accurately in the care plan.
- Update care notes in the patient's home and update the Hospice of St Francis electronic care records accurately and in a timely fashion.
- Be responsive and flexible in meeting the needs of patients and families within the agreed care plan.
- Care certificate/NVQ qualified level 2 or equivalent.
- 3 years' experience in a similar role, with the competence to deliver direct and intimate personal care.
- Current experience delivering care as part of a rostered team, working 24/7, 365 days a year.
- Knowledge of and experience in care at the end of life and after death.
- Current driving license, with use of own car.
