About the Role
We are seeking a highly motivated and experienced Mental Health Therapist to join our team at Sheffield Health and Social Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will work closely with service users, carers, and other healthcare professionals to provide high-quality care and support.
Key Responsibilities
- Act as a named worker for a designated caseload, providing a consistent approach and a named person to contact.
- Develop a rapport with service users within appropriate boundaries, promoting their independence and well-being.
- Encourage service users to be fully involved and engaged with their agreed care plan, accessing appropriate services as needed.
- Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional.
- Monitor service users' progress, level of functioning, and mental state, alerting the appropriate staff involved in their care to any early signs of relapse.
- Take a role in evaluating the care provided and identifying opportunities for improvement.
- Support carer needs as required and participate in health promotion, including physical health needs.
- Participate in and help run social activity groups, promoting a sense of community and social connection among service users.
Requirements
- A caring and confident attitude, with the ability to work in a multidisciplinary environment.
- Excellent written and verbal communication skills, with the ability to respond to the individual rights/needs/opinions of service users.
- Able to highlight risks in community settings, including those associated with lone working.
- Basic IT skills, with the ability to read, understand, and follow care pathways, and to write and record accurate information.
- An interest in and commitment to ongoing development in the role, with the ability to identify personal development needs and participate in appraisal and clinical supervision.
