Care Coordinator Job Summary:

We are seeking a highly motivated and compassionate individual to fill the role of Care Coordinator at David Lewis. As a key member of our team, you will play a vital role in supporting our residents and ensuring that their care and support needs are met.

Your primary responsibility will be to coordinate and deliver care and support services to our residents, ensuring that they receive high-quality care and support in a safe and welcoming environment. You will also be responsible for developing and implementing care plans, collaborating with other team members, and managing our day-to-day activities.

Key Responsibilities:

  • Coordinate and deliver care and support services to our residents, ensuring that their needs are met.
  • Develop and implement care plans, in consultation with our residents and other team members.
  • Collaborate with other team members to ensure that our home remains a safe and welcoming environment.
  • Manage our day-to-day activities, including assisting with personal care and participating in activities.

Requirements:

  • Right to work in the UK.
  • Full UK driving license, as driving is an essential part of the role.
  • Proactive and organised, with excellent time management skills.
