Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking a highly skilled and experienced Community Nursing Sister to join our team in Wokingham, Berkshire. As a key member of our Community Nursing team, you will play a vital role in providing high-quality care to patients in their own homes.

Main Responsibilities
  • Provide leadership and management to a team of community nurses, ensuring the delivery of excellent patient care and outcomes.
  • Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals to achieve the best possible results.
  • Conduct regular assessments and reviews of patient care, identifying areas for improvement and implementing changes as needed.
  • Collaborate with other healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Recruit, train, and develop staff, promoting a culture of excellence and continuous improvement.
  • Manage and maintain accurate and confidential patient records, adhering to Trust policies and procedures.
Requirements
  • Registered Nurse - Adult qualification and NMC registration.
  • Active on the Mentor Register.
  • Evidence of leadership and management development.
  • Recognised coaching qualification or willingness to undertake.
  • At least 2 years' experience in community nursing at Band 6.
  • Experience of leadership within a team.
Desirable Criteria
  • Specialist Practitioner qualification (DN Degree, diploma, or Certificate in District Nursing).
  • Non-Medical Prescriber/Nurse Prescriber Qualification.
