We are seeking a highly motivated and experienced nurse to join our heart failure service in Derbyshire. As a Heart Failure Nurse/Heart Failure Specialist Nurse, you will play a crucial role in delivering high-quality care to patients with heart failure, working closely with our team to provide comprehensive support and education.
Main ResponsibilitiesThe successful candidate will be responsible for:
- Managing a caseload of heart failure patients to ensure their treatment is optimized for the best possible quality of life.
- Delivering community-based care, including assessment, care, and treatment of heart failure patients.
- Working closely with local community services, GPs, cardio-renal services, and tertiary providers to ensure seamless care coordination.
- Providing education and support to patients and their families to empower them to manage their condition effectively.
- Participating in multidisciplinary meetings and contributing to the development of the heart failure service.

RequirementsTo be successful in this role, you will need:
- Registration as a Registered General Nurse (RGN) with the NMC.
- Teaching and assessing in clinical practice qualification (e.g., ENB 998, C&G 7307, or equivalent).
- Experience in community nursing, with a focus on heart failure care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a flexible approach to working hours.
