Service Care Solutions Ltd is seeking an experienced Advanced Care Practitioner to join our team in Market Drayton. As a Primary Care Mental Health Professional, you will play a crucial role in providing high-quality care to patients with mental health needs.

Key Responsibilities:
  • Provide triage and assessments of mental health needs using evidence-based outcome measures.
  • Work in partnership with service users to identify effective strategies for managing mental health problems.
  • Offer information, advice, education, and health promotion to service users, carers, and the wider community.
  • Critically analyze service user information and formulate individual plans of care using evidence-based brief interventions.
Requirements:
  • Must have GP Practice/Primary Care experience and knowledge of using EMIS & Digital Badge.
