Key Responsibilities:- Provide triage and assessments of mental health needs using evidence-based outcome measures.
- Work in partnership with service users to identify effective strategies for managing mental health problems.
- Offer information, advice, education, and health promotion to service users, carers, and the wider community.
- Critically analyze service user information and formulate individual plans of care using evidence-based brief interventions.
- Must have GP Practice/Primary Care experience and knowledge of using EMIS & Digital Badge.
