We are seeking a dedicated and skilled professional to join our team at Charles Hunter Associates as an Adult Social Services Coordinator. This role involves working closely with healthcare professionals to develop care plans that meet the needs of adults in hospital.

About the Job:

The successful candidate will have a degree in social work or a related field and a minimum of 1 year's post-ASYE experience. They will also have experience working within safeguarding hubs, hospitals, communities, and with older people and physical disabilities.

Salary: £45,000 - £55,000 per annum (dependent on experience)

Key Responsibilities:

  • Develop and implement care plans that meet the needs of adults in hospital
  • Work closely with healthcare professionals to ensure effective discharge
