About the Role

We are seeking a highly skilled and dedicated Nurse Care Manager to join our team in Leeds. As a key partner with the National Development Team for Inclusion, we have led on developing the strength-based approach to social care delivery. Our team is committed to increasing integration for better services supporting people with learning disabilities.

The successful candidate will be responsible for providing a high-quality and comprehensive service to learning disabled adults, their carers, and community in accordance with organisational needs, policy, practices, and within statutory guidelines. This will involve carrying a caseload, undertaking assessments, and developing service support plans that correspond to assessed needs of service users, families, and carers.

Key responsibilities include:

  • Carrying a caseload and undertaking assessments of service users and carer needs using a person-centred approach
  • Maintaining and updating case notes and other records, providing concise and informative reports
  • Working in the court arena with appropriate support to obtain and implement court orders and give evidence in court as required
  • Contributing to planning and developing future services and policies, promoting innovative ways of developing support to service users and supervising students
  • Developing and maintaining effective working relationships with professionals internally and externally to ensure an integrated, holistic, and multidisciplinary approach to care management and safeguarding

Requirements include:

  • Appropriate professional qualification (Registered LD nurse or dual LD Nurse/Social worker qualification) and registered with the Nursing and Midwifery Council upon commencement in post and maintain registration
  • Minimum of 12 months post-qualification experience of working in a statutory/health setting
  • Knowledge of current trends, policies, legislation, and regulatory bodies in relation to vulnerable people within a social care setting
  • Demonstrate an awareness of needs and safeguarding issues associated with vulnerable people
  • Demonstrate understanding of diversity issues within local communities
