Job Title: Home Care Professional
Description:
We are seeking a compassionate and dedicated individual to join our team as a Home Care Professional. As a valued member of our community, you will provide personalized care and support to individuals in their own homes, promoting independence, dignity, and well-being.
Key Responsibilities:
- Deliver high-quality care and support services to clients in their homes.
- Promote independence and autonomy through enabling approaches.
- Support clients to meet their outcomes, involving families and professionals as needed.
- Connect clients to their communities, accessing local support and services.
- Document and report client information accurately and effectively.
Requirements:
- Able to communicate clearly and empathetically with clients and their families.
- Flexible and able to work varied shifts, including evenings and weekends.
- Committed to maintaining confidentiality and adhering to professional boundaries.
