Job Title: Home Care Professional

Description:

We are seeking a compassionate and dedicated individual to join our team as a Home Care Professional. As a valued member of our community, you will provide personalized care and support to individuals in their own homes, promoting independence, dignity, and well-being.

Key Responsibilities:

  • Deliver high-quality care and support services to clients in their homes.
  • Promote independence and autonomy through enabling approaches.
  • Support clients to meet their outcomes, involving families and professionals as needed.
  • Connect clients to their communities, accessing local support and services.
  • Document and report client information accurately and effectively.

Requirements:

  • Able to communicate clearly and empathetically with clients and their families.
  • Flexible and able to work varied shifts, including evenings and weekends.
  • Committed to maintaining confidentiality and adhering to professional boundaries.
