About the Job:
We are seeking a highly skilled Patient Care Coordinator to join our team at Barchester ApplyG CPA. As a Patient Care Coordinator, you will play a vital role in ensuring that our residents receive the highest level of care and support.

Your main responsibility will be to coordinate the care of our residents, working closely with other healthcare professionals to ensure that their needs are met. You will also be responsible for developing and implementing individualized care plans, which will be tailored to the specific needs of each resident.

To be successful in this role, you will need to have excellent communication skills, a strong understanding of the care sector, and the ability to work effectively in a fast-paced environment. You will also need to have current NMC registration and experience in producing care plans and risk assessments.

The salary for this position is £34,000 - £44,000 per annum, depending on experience. This is a competitive salary that reflects the importance of this role in delivering high-quality patient care.
