About the Job

We are seeking a skilled Care Support Professional to join our team at HC-One. As a key member of our care team, you will play a critical role in delivering high-quality care to our residents. Your primary responsibility will be to provide supportive care services that enable our residents to live fulfilling lives.

The ideal candidate will have a strong commitment to providing person-centered care, with a focus on promoting independence, dignity, and respect for all residents. You will work collaboratively with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.

**Job Summary:**

  • Provide supportive care services to residents, including assistance with daily living activities
  • Develop and implement individualized care plans in collaboration with the care team
  • Participate in resident assessments and care planning meetings
  • Contribute to a positive and respectful care environment

**Requirements:**

  • Strong commitment to providing person-centered care
  • Excellent communication and interpersonal skills
  • Able to work collaboratively as part of a team
  • Ability to prioritize tasks and manage time effectively
