About the Role
We are seeking an experienced and skilled Discharge Coordinator to join our team at Surrey Downs Health and Care. As a key member of our multidisciplinary team, you will play a vital role in ensuring the timely and safe discharge of patients from our community hospital setting.
Key Responsibilities
- Work closely with the ward team to facilitate the effective and appropriate discharge of patients, liaising with social and health service colleagues, attending weekly MDT meetings and ward handovers.
- Develop and implement innovative discharge practices within the ward environment, ensuring best practice in planning safe and timely patient discharges.
- Take responsibility for ensuring all necessary arrangements are in place for discharge, including TTOs, GP discharge letters, outpatient appointments, and patient transport arrangements.
- Act as the focal point of contact for up-to-date information in relation to discharge communication, providing good quality written discharge information to patients and carers.
