Job Summary

Surrey Downs Health and Care is seeking a skilled Discharge Coordinator to manage community hospital discharges, ensuring timely, appropriate, and safe patient discharges.

This role involves coordinating relevant services, liaising with social and health service colleagues, and attending weekly MDT meetings and ward handovers.

The successful candidate will be responsible for assessing, planning, implementing, and evaluating care programs with minimal supervision.

Main Duties

• Work closely with the ward team to facilitate timely patient discharges

• Liaise with social and health service colleagues to ensure seamless care transitions

• Attend weekly MDT meetings and ward handovers to coordinate discharge planning

Job Responsibilities

• Assess, plan, implement, and evaluate care programs with minimal supervision

• Work as a member of the Multi-Disciplinary Team without direct supervision

• Promote a high standard of evidenced-based nursing care and dignity for patients

• Assist senior nurses with ensuring patients' nutritional status is maintained and maximized

• Assist senior nurses with ensuring a clean ward environment and best practice in infection control
