Job Overview

We are seeking a skilled and compassionate Care Team Manager to join our team at Home Instead. As a key member of our care services team, you will be responsible for delivering exceptional care to our clients in the comfort of their own homes.

About the Role

The successful candidate will have a strong background in health and social care, with experience in supervising care professionals and managing client relationships. You will be passionate about delivering high-quality care and committed to helping our clients live independently and happily in their own homes.

Responsibilities

  • Conduct regular care professional support visits and complete ongoing competencies.
  • Create, update, and audit client care plans, ensuring they align with our high standards.
  • Support clients and their families through consultations, client enquiries, and the management of the care journey.
  • Maintain accurate records of client and care professional interactions on Home Instead software.
  • Coordinate client introductions and build positive relationships with clients, their families, and other professionals involved in their care.
  • Support and mentor care professionals, promoting a culture of excellence and teamwork.
  • Schedule day-to-day changes to support short-notice sickness, ensuring continuity of care.
  • Engage in regular contact with clients and care professionals to ensure satisfaction and identify areas for improvement.
  • Participate in on-call duties as required, demonstrating flexibility and commitment to our clients' needs.

Requirements

  • A Level 3 NVQ in Health and Social Care or equivalent qualification.
  • Proven experience in the care sector, delivering a wide range of personal care services.
  • Experience working with MAR sheets and medication in a supervisory role.
  • Possess good communication skills, with the ability to build rapport quickly.
  • Confident in using care management technology, including providing support and training to care professionals.
  • Able to maintain confidentiality, adhering to current legislation.
  • Familiarity with IT systems, particularly Microsoft Office or Google Suite, and virtual communication platforms.
  • In possession of a full UK driving license and means of transport, if required within the territory, to visit clients.

  • Willingness to carry out client visits as required.

Salary Information

The salary for this position is £24,250 per annum, reflecting the importance of this role in delivering exceptional care to our clients.
