Job Description:

The Referrals Hub Clinical Specialist Lead will be responsible for managing the referral process, ensuring seamless transitions between services, and enhancing the overall patient experience.

You will work closely with multidisciplinary teams, coordinating efforts to provide cohesive care and support.

Your expertise will be instrumental in developing and implementing service redesign initiatives, addressing operational challenges, and promoting quality improvement across the organization.

Responsibilities:

  • Lead the Referrals Hub team by example, demonstrating strong leadership skills, empathy, and professionalism
  • Manage and coordinate referrals, ensuring timely and effective responses to patient needs
  • Develop and maintain relationships with external partners, healthcare providers, and stakeholders
  • Contribute to strategic planning, goal-setting, and performance monitoring

Requirements:

We seek an experienced professional with a background in nursing or AHP roles, ideally within the NHS or Local Government sector. The ideal candidate will possess excellent communication, interpersonal, and problem-solving skills, with the ability to work independently and collaboratively within a fast-paced environment.
