We are seeking a skilled and compassionate Family Support and Wellbeing Officer to join our team at Polaris Community. As a key member of our family preservation service, you will play a vital role in delivering high-quality services to children, young people and families, promoting equality, diversity and rights for all individuals associated with the service.

Your primary focus will be on holding case responsibility, overseeing and supporting the delivery of intervention services to children, young people and families programme, responding to referrals or service requests from Local Authority partners, and implementing, monitoring and reviewing service plans relating to the assessed needs of children, young people and their families, in partnership with other agencies.

Responsibilities:

  • Hold case responsibility, oversee and support the delivery of intervention services to children, young people and families programme
  • Respond to referrals or service requests from Local Authority partners
  • Implement, monitor and review service plans relating to the assessed needs of children, young people and their families, in partnership with other agencies
  • Refer children, young people and their families to appropriate specialist or universal services in order that their needs are met

Requirements:

To be successful in this role, you will need to hold a Social Work qualification recognised by Social Work England or equivalent body, and have proven experience of working with children, young people and their families (including foster parents) with complex and multiple needs, including child protection issues.
