Senior Nurse Practitioner

This exciting opportunity has arisen for a highly motivated and experienced Senior Nurse to join our team at Oxleas NHS Foundation Trust. As a Senior Nurse, you will be responsible for delivering high-quality primary care services and specialist interventions for offenders at HMP Erlestoke.
Key Responsibilities- Develop and implement effective patient-centered care planning tools and long-term condition registers.
- Ensure timely delivery of clinics and consultations in a clinically effective manner.
- Implement strategies designed to promote and improve health and prevent disease while in prison.
- Support the development of evidence-based practice in the specialist field and promote research as appropriate.
- Postgraduate qualification or equivalent experience.
- Minimum of two years' experience at Band 5.
- Working knowledge of current issues/agendas facing prison/offender healthcare.
- Experience of multi-professional collaboration including working in partnership with other statutory and voluntary organisations.
