About the Role

The successful candidate will be responsible for assessing patients, planning treatment plans, and coordinating support and equipment to meet the patients' needs. This will involve working across both rehabilitation and ESD services, promoting continuity of care into the community setting and collaborating with the wider Multi-Disciplinary Team (MDT) to support long-term rehabilitation pathways for patients in their own homes.

  • Assessing patients to identify their needs and develop effective treatment plans.
  • Coordinating support and equipment to meet patient needs.
  • Collaborating with the MDT to ensure seamless care coordination.
