About the Role

We are seeking a dedicated and compassionate individual to join our team as a Support Worker. As a Support Worker, you will play a vital role in supporting adults with learning disabilities and autism to live independently and achieve their goals.

Key Responsibilities

You will be allocated a caseload of individuals who require support with daily living tasks, personal care, and community integration. Your role will involve:

  • Providing person-centered support to individuals with learning disabilities and autism
  • Assisting with daily living tasks, such as personal care, meal preparation, and household chores
  • Supporting individuals to access community resources, such as education, employment, and leisure activities
  • Collaborating with multidisciplinary teams to develop and implement individualized support plans
