About the Role:
The Field Care Supervisor is a key member of our team, responsible for delivering high-quality care support services in the community. As a Field Care Supervisor, you will work closely with Care Coordinators to plan and implement effective care worker rotas, ensuring that our clients receive the best possible care.
Key Responsibilities:
- Conduct regular reviews of individual services, including risk assessments and needs evaluations.
- Work with Care Coordinators to develop and implement care plans that meet the unique needs of each client.
- Provide on-site supervision and support to care workers, ensuring they have the skills and knowledge to deliver high-quality care.
- Participate in multi-disciplinary reviews of client needs, working with healthcare professionals and other stakeholders to ensure seamless care delivery.
- Shadow new care workers to ensure they are competent and confident in their roles.
Requirements:
- Experience in conducting initial risk and needs assessments for individual service users.
- Ability to work in partnership with service users, their families, carers, and healthcare professionals to deliver holistic care packages.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
- Ability to work independently and as part of a team, with a flexible approach to work.
