Social Care Specialist

We are seeking a highly skilled Social Care Specialist to join our Adult Mental Health Team (AMHT) in Oxfordshire, Buckinghamshire, Swindon, Wiltshire, Bath and North East Somerset.
About the Role- As a key member of the AMHT, you will work alongside multidisciplinary professionals to ensure that individuals receiving support have their full care and support needs assessed under the Care Act 2014.
- You'll play a crucial role in coordinating support plans that address these needs, empowering service users and their families to make meaningful decisions about care.

Responsibilities- Manage care effectively in conjunction with the Multidisciplinary Team, taking account of each member's contribution to the care package.
- Able to work positively with clients or carers who express verbal concerns or complaints about the service and attempt early and effective resolution in conjunction with the PALS Service.
- Show awareness of local Trust policies and adhere to policy and procedural guidance.
Estimated Salary: £28,000 - £40,000 per annum
