Community Outreach Worker

Job Summary: We are seeking a skilled and compassionate Community Support Worker to join our team. The successful candidate will provide supportive care to adults with mental health needs in their own homes or community settings.
Key Responsibilities:
- Carry out assessments to identify service users' needs and goals
- Develop and implement individualised care plans in collaboration with other healthcare professionals
- Provide emotional support and practical assistance to service users
- Assist service users with daily living activities, such as personal care and household tasks
Requirements:
- Diploma in Health and Social Care or equivalent qualification
- Good standard of general education, including numeracy and literacy skills
- Recent further development/training in health and social care
- Commitment to Continuing Personal Development
