About the Role

We are seeking an experienced and skilled Primary Care Lead to join our team at Central and North West London NHS Foundation Trust. As a Primary Care Lead, you will be responsible for the operational management of primary care services at HMP Aylesbury, ensuring an equitable service to that in the community.

Key Responsibilities
  • Provide clinical leadership and direction to the primary care team, ensuring the delivery of high-quality, evidence-based care.
  • Develop and implement policies and procedures to ensure the effective management of primary care services.
  • Work collaboratively with other agencies to ensure the seamless delivery of care to patients.
  • Manage the primary care team, including staff employed by different provider organizations or those working within healthcare through contract management arrangements.
  • Ensure the highest quality, evidence-based service is provided, and that the service is delivered in a flexible and efficient manner within allocated resources.
  • Develop and evaluate the healthcare provision, working effectively with others to achieve this goal.
  • Lead on other portfolio responsibilities connected to the development of the service, working collaboratively with key partners.
  • Responsibility for security within the Healthcare facility and any other areas of practice within the prison.
  • Responsibility for prisoner care as a specialist Healthcare Practitioner for Primary Care and Public Health.
Requirements
  • Registered General Nurse with significant professional experience in Primary Care.
  • Management qualifications and evidence of continuous professional development, practice teaching, mentoring.
  • Experience working at Band 7 Nurse and experience of supervising staff.
  • Experience of joint working with other agencies and experience of working in secure settings.
  • Knowledge of Chronic Disease and Long-Term Condition Management, and knowledge of the principles of Best Value/Clinical Governance.
