About the Role

We are seeking a highly skilled and experienced Clinical Lead to join our team at Central and North West London NHS Foundation Trust. The successful candidate will be responsible for leading the Primary Care provision at HMP Aylesbury, ensuring an equitable service to that in the community.

Key Responsibilities

  • Operational management of Primary Care provision
  • Clinical leadership in primary care and public health
  • Deputising for the Head of Healthcare when required
  • Ensuring the highest quality, evidence-based service is provided
  • Coordinating healthcare provision across the whole prison establishment
  • Managing the service within allocated resources
  • Developing and evaluating healthcare provision through effective leadership and partnership working
  • Leading on other portfolio responsibilities connected to the development of the service
  • Responsibility for security within the Healthcare facility and other areas of practice within the prison
  • Responsibility for prisoner care as a specialist Healthcare Practitioner for Primary Care and Public Health
  • Management of the primary care team, including staff employed by different provider organisations or those working within healthcare through contract management arrangements

Main Duties

  • Leading the use of evidenced-based care in negotiating, assessing, planning, and evaluating patient healthcare
  • Developing and managing specialist clinics for the management of Chronic Disease
  • Leading and managing the healthcare response to medical emergencies within the prison
  • Seeking specialist advice from other healthcare professionals to develop comprehensive care packages when needed
  • Undertaking and supervising assessment of suitability of patients to be transferred or released
  • Ensuring comfort and personal appearance, hygiene, and nutritional needs of patients are met
  • Using specialist knowledge and expertise to make clinical decisions and influence the decisions of other Healthcare professionals
  • Undertaking and supervising triage to determine clinical priorities and potential need to admit to the Healthcare Inpatient Facility
  • Respecting patient individuality and maintaining their privacy within the constraints of the custodial environment
  • Supervising the support of patients to address issues affecting their health, behaviour, and social welfare
  • Liaising with prison officers and external Healthcare providers regarding the discharge pathway of patients from Healthcare
  • Developing evidenced-based clinical policies and procedures
