About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Live Connected. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with mental health, learning disabilities, and dementia.

Key Responsibilities:

  • Undertake daily living tasks as agreed with service users, their families, and professionals involved in their care plan.
  • Work within the policies and procedures of Connected Living, ensuring the safety and well-being of service users.
  • Complete Health and Safety checks and ensure safe medication is delivered and ordered to service users' homes.
  • Provide opportunities for service users to express their preferences and maintain their independence.
  • Manage service users' behaviors in the least restrictive way possible and ensure all daily notes, handovers, and documentation are completed by the end of shift.

Requirements:

  • Previous experience in a care or support role.
