About the Role

We are seeking a compassionate and dedicated individual to join our team as a Support Worker. As a Support Worker, you will play a vital role in providing care and support to individuals in our community, enabling them to live independently and make their own choices.

Key Responsibilities
  • Provide person-centred care and support to individuals, promoting their independence and well-being
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores
  • Support individuals to access community resources and services, such as healthcare, education, and leisure activities
  • Develop and maintain positive relationships with individuals, their families, and other professionals
  • Contribute to the development and implementation of care plans, ensuring they are tailored to individual needs and preferences
