About the Role
We are seeking a highly skilled and experienced Nurse Practitioner to join our PATCH team at The Hillingdon Hospital NHS Foundation Trust. As a key member of our team, you will provide high-quality, patient-centered care to children and families in the community.
Key Responsibilities
- Provide nurse-led care through telephone reviews and home visits for children who would have otherwise been admitted to hospital.
- Deliver health education and promotion to children and their families at home during acute episodes of illness to reduce re-attendances and build resourcefulness within the community.
- Work closely with the lead nurse/practitioner, consultant, and multi-disciplinary team members to monitor and maintain policies, standards of nursing care, and staff development.
- Be flexible in your approach to the clinical and community area and provide support for colleagues, junior staff, and students.
