We are seeking a highly motivated and experienced Care Practitioner to join our team at Lincolnshire County Council. As a Care Practitioner, you will play a vital role in providing excellent care and support to our service users, ensuring they receive the best possible service.

As a Care Practitioner, you will work closely with our team to develop and implement individual care plans, ensuring that our service users receive the best possible support. You will also be responsible for maintaining accurate records and reporting any concerns or issues to the relevant authorities.
