We are seeking an experienced and skilled Health and Wellness Improvement Programme Manager to join our team at Cwm Taf Morgannwg University Health Board. The successful candidate will play a key role in the development and delivery of specific primary care projects and programmes, and will be responsible for driving and developing the prevention agenda.
Key Responsibilities- Lead and redesign wellness and prevention services across primary care to include long-term condition management.
- Undertake local service evaluations on work streams.
- Provide leadership support and drive for continuous improvement of primary care teams.
- Develop and implement policies and procedures that impact across programmes.
- Manage dedicated project resources and resources contributing to project delivery.
- Support programme leads and senior responsible officers in the development and implementation of key objectives.
- Apply high levels of communication and change management skills to overcome resistance to change.
- Assist programme leads and senior responsible officers in reviewing the performance of projects.
- Highly developed knowledge of project management methodologies and techniques, including change management and service redesign.
- In-depth specialist knowledge and expertise across a range of disciplines, including performance management, project management, quality, and service improvement tools and techniques.
- Proven work record of consistently achieving high standards and delivering objectives and priorities.
- Successful track record of leading and delivering large-scale, complex projects.
