Gray Healthcare is seeking a dedicated and compassionate Support Worker to join our team in Lincoln. As a Support Worker, you will play a vital role in supporting an individual with Treatment Resistant Schizophrenia, COPD, and Heart Failure, who requires 1:1 support 24 hours a day.

Key Responsibilities

  • Provide social care experience, ideally within mental health, and support individuals with complex health needs and Learning Disabilities.
  • Implement detailed care plans and risk assessments to ensure the individual's safety and well-being.
  • Support the individual in their daily activities, promoting independence and autonomy.
  • Work collaboratively with the multi-disciplinary team to provide a comprehensive support package.
  • Participate in ongoing training and development to enhance your skills and knowledge.

Requirements

  • Current Right to Work in the UK and at least 6 months of employment in the UK.
  • Enhanced DBS check and satisfactory references.
