Key Responsibilities- Provide direct social work services to vulnerable citizens, adhering to statutory, national, and regional policies and guidelines.
- Work collaboratively with colleagues, private sector organizations, and health services to deliver an integrated response to meeting the needs of vulnerable populations.
- Build effective relationships with citizens, families, and carers to identify and meet their needs, ensuring their rights are safeguarded.
- Ensure timely completion of assessments and reviews, adhering to statutory timescales.
- Employ person-centered planning approaches to support citizens in assessment, care planning, and review processes.
- Respond to safeguarding concerns in accordance with the Wales Interim Policy and Procedure for POVA.
- Conduct POVA investigations and complete risk assessments.
- Contribute to the preparation and presentation of reports and plans at POVA Strategy meetings.
- Work in partnership with Community Wellbeing services to develop knowledge and awareness of available resources.
- Maintain professional accountability for resource allocation, adhering to service criteria, legislation, and financial regulations.
- Ensure an early and effective response to concerns or complaints from citizens or other agencies.
- Support citizens and their carers/families to actively participate in service development.
- Prepare for and contribute effectively to meetings, chairing meetings as required.
- Prepare written reports for various purposes, ensuring accuracy, coherence, and completeness.
- Attend team meetings and contribute to service development.
- Maintain a specific caseload of varied complexity.
- Utilize research, theory, evidence-based practice, and problem-solving methodologies to achieve good outcomes for citizens.
- Maintain good recording practices, ensuring records are accurate, coherent, and up-to-date.
- Ensure casework is effectively planned and outcome-focused, with citizens, families, and carers fully represented.
- Ensure timely completion of tasks and respond promptly to communications.
- Promote anti-discriminatory practices and act in a non-oppressive manner towards service users, carers, and colleagues.
- Provide a positive role model in relation to conduct within the workplace, including good workload and time management.
- Exercise responsibility under the Health and Safety at Work Act and respond promptly and robustly to these regulations.
- Assist with the duty system as needed and undertake backup duties on a regular basis.
- Transport/accompany citizens to appointments, as appropriate, following risk assessments.
- Maintain professional registration and comply with post-registration training and learning requirements.
- Prepare for and participate in professional supervision and performance development reviews to support personal development and adherence to professional standards.
- Attend training and seminars to develop personal and professional expertise, disseminating knowledge and learning to colleagues as appropriate.
- Establish good working relationships internally and externally.
- Comply with the Authority's Policies and Procedures, reporting any inadequacies to Senior Officers.
- Participate actively in supporting the Authority's principles and practices of equality of opportunity.
- Apply Health & Safety practices in daily operations, sharing responsibility for Health & Safety across the department, directorate, and Authority.
- Provide information for personnel, administrative, and payroll purposes.
- Undertake any other reasonable tasks, as determined by your Line Manager or Head of Service.
- Be aware of the Corporate Safeguarding Policy and report concerns in the appropriate manner and timescales.
