**Job Overview**

We have an exciting opportunity for a **Healthcare Rehabilitation Specialist** to join our multidisciplinary team. As a key member of our ARCH service, you will play a vital role in delivering rehabilitation services to inpatients and community patients.

**Key Responsibilities**

To deliver rehabilitation services within a multidisciplinary team to ensure positive clinical outcomes and effectiveness by providing a coordinated service including continuity and quality of care.

To support patient flow from hospital to their own homes by giving appropriate information, advice, and education.

**Requirements**

A full UK driving licence and access to a car.

Relevant NVQ Level 3 or equivalent.

12-18 months experience in health or social care across community and acute settings.

Displays an understanding of HDFT KITE values.

Experience of rehabilitation/reablement/promotion of patient independence.

Knowledge of home first/D2A.

Understanding of medicines management.
