Care Field Supervisor

Key Responsibilities:- Support the delivery of care services by conducting assessments and reviews of individual services, and providing practical supervision, instruction, and support to care workers.
- Work closely with Care Coordinators to plan realistic and effective care worker rotas, taking into account the unique needs of each individual and the skills and attributes of the care worker.
- Carry out regular reviews of individual services, including repeated and renewed needs and risk assessments.
- Participate in multi-disciplinary reviews of service users' needs, working collaboratively with healthcare professionals and other stakeholders.
- Shadow new care workers to ensure they're competent in their roles.
- Conduct on-site observations and evaluations of care worker practice as part of routine supervision, or in response to specific performance concerns.
As a Care Field Supervisor, you'll spend most of your time in the community, delivering care services to individuals in their own homes. However, you'll also be required to perform some administrative tasks in the office.
Requirements:- Experience in conducting initial risk and needs assessments for individual service users, either in their homes or in other settings such as hospitals or residential/nursing homes.
- Ability to work in partnership with service users, their families, carers, advocates, healthcare professionals, and others to ensure seamless delivery of holistic packages of care support.
