Job Title: Field Care Supervisor

We're seeking an experienced and compassionate Field Care Supervisor to join our team. As a Field Care Supervisor, you will play a vital role in ensuring the seamless delivery of care services to our clients, working closely with our care workers and care coordinators to provide exceptional support.

Key Responsibilities:

  • Support the planning and delivery of care services, including the assessment and review of individual services and the supervision of care workers.
  • Work collaboratively with care coordinators to develop and implement effective care plans, taking into account the unique needs and preferences of each client.
  • Conduct regular reviews of individual services, including risk assessments and needs assessments, to ensure the highest standards of care are maintained.
  • Provide guidance and support to new care workers, ensuring they are equipped to provide high-quality care and support.
  • Participate in multi-disciplinary reviews of service users' needs, working closely with healthcare professionals and other stakeholders to ensure comprehensive care plans are in place.

Requirements:

  • Experience in carrying out initial risk and needs assessments, either in the client's home or in other settings such as hospitals or residential/nursing homes.
  • Ability to work in partnership with clients, their families, carers, advocates, and healthcare professionals to ensure seamless delivery of holistic packages of care support.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, care workers, and other stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
