About the Role

We are seeking an experienced mental health professional to join our Crisis Resolution and Home Treatment Team as a Community Mental Health Practitioner. This is an exciting opportunity to work in a dynamic and supportive team, providing high-quality care to individuals experiencing acute mental health crises.

Key Responsibilities
  • Provide urgent assessments and intensive community-based treatment services to individuals in acute crisis, as an alternative to inpatient admission.
  • Work collaboratively with the multidisciplinary team to provide safe and effective treatment in the community, and facilitate early discharges from acute inpatient units.
  • Develop and implement effective team systems and processes for efficient and effective team working, including outcome measures and audit.
  • Provide leadership and supervision to the team, ensuring the highest standards of care are maintained.
Requirements
  • Post-registration experience with at least some experience at band 6.
  • Experience in acute and community areas, with a focus on care planning, change management, and use of Microsoft Office.
  • Knowledge of physical health contributions to mental health issues, difficulties, and assessment.
  • RMN or Dip/BSc (Hons) in Mental Health Nursing, with desirable qualifications including ENB 998/Preceptorship and Mentorship, and ENB 812/BSc (Hons) Specialist Community Nursing Practice (Mental Health).
