We are seeking an experienced and skilled Neurological Nurse Specialist to join our Community Neurology Team at NELFT NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with neurological conditions, including Multiple Sclerosis, Parkinson's Disease, and Motor Neurone Disease.

Key Responsibilities:
  • Develop and implement individualized care plans for patients with neurological conditions
  • Provide education and support to patients and their families on disease management and self-care
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Conduct assessments and evaluations to identify patient needs and develop strategies for improvement
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements:
  • Registered Nurse qualification (Part 1 of NMC register)
  • Experience working in community neurology or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
