About the Role

We are seeking an Advanced Clinical Practitioner to join our team at Shropshire Community Health NHS Trust. As an Advanced Clinical Practitioner, you will be responsible for assessing and treating patients in crisis, working collaboratively with other clinicians and organisations to meet their needs. You will also be expected to develop and implement programmes of care, under the direction of medical and ACP leads. With your advanced clinical practice skills and experience in supervision or management, you will play a key role in ensuring the delivery of safe patient care.

Key Responsibilities
• Work alongside pre-existing Urgent Care Response (UCR) and Planned Care workstreams to deliver a seamless transition through services
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
• Prescribe or recommend initial medication, appropriate to patient needs and in accordance with evidence-based practice

Requirements
• Current registration with NMC or HCPC without restrictions on practice
• Non-medical prescriber (V300)
• Demonstrable clinical supervision training or management experience or qualification
• Demonstrable recent Urgent Community Response, Acute or Emergency Care experience
• Experience of supervision or management of junior staff

Salary and Benefits
We offer a competitive salary of £43,041 - £67,399 per annum, depending on experience, plus a range of benefits including generous annual leave entitlement, pension scheme, and opportunities for career development.
